Chart intake by active ingredient instead of medication name #2

/medications/history now returns each dose's ingredient breakdown
Chart aggregates amount = per-unit ingredient amount x pills taken,
keyed by normalized ingredient name
Per-ingredient units (mg/mcg/IU) shown in legend and tooltip
Chart defaults to empty; user selects which ingredients to view
Meds without ingredient rows are excluded, with a hint to add them
Scan now always populates at least one active ingredient
Ingredient editor labeled "Active ingredients" for Rx/OTC,
"Ingredients" for supplements
